FAQS
For simple procedures, we can usually accommodate patients. However, for more complex procedures, patients with complicated medical histories or patients desiring I.V sedation, usually consultations are required first.
Generally, a consultation appointment is approximately 30 minutes. This would include pre-appointment paperwork, exam and consultation by Dr. Clemens and this will also be an opportunity for Dr. Clemens to answer all of your questions regarding your care.
We take most insurances, however due to the complexity of certain plans, you will need to speak to the front office to determine whether or not we participate in your plan or accept your insurance.
Dr. Clemens has offices in Cedar Hills and Forest Grove. He is in the office five days a week, Monday through Friday. His hours are 8:00 am to 4:00 pm, Monday through Thursday and 8:00 am to 3:00 pm on Fridays.
